Cancer drug price cuts under the National Pharmaceutical Pricing Authority’s (NPPA) trade margin cap take effect on 9 November 2026, the Ministry of Chemicals and Fertilizers said on 7 November. Revised maximum retail prices (MRPs) for 137 non-scheduled anti-cancer drugs are expected to save patients about ₹3,502.31 crore a year.

The cap, notified in the Gazette on 17 October 2026 under Paragraph 19 of the Drugs (Prices Control) Order (DPCO), 2013, limits the trade margin on these medicines to 30% of the MRP. It follows the NPPA’s earlier revised MRP list for 543 cancer drug brands published in late October.

How much cancer drug prices will fall

According to the ministry’s statement, 76 pharmaceutical companies have so far reported revised MRPs for 1,225 anti-cancer formulations covering 121 of the 137 drugs. The figures show how deep the cuts run across the market:

  • MRP reductions reported in 1,031 formulations.
  • 600 brands cut prices by more than 50%.
  • The highest single reduction is 96.45%.
  • Estimated annual consumer savings of ₹3,502.31 crore, up from ₹3,357.49 crore estimated on 4 November.

Between 10 and 15 of the listed drugs are yet to be launched in India. The ministry said the 30% margin cap will apply to them as and when they reach the market.

What the trade margin cap means for pharma companies

New MRPs are worked out from each product’s average price to stockist or distributor in March 2026, with the trade margin capped at 30% and GST added on top. Companies can still take the annual price increase allowed under Paragraph 20(1) of the DPCO, provided the 30% margin limit continues to hold. The NPPA has already issued compliance FAQs saying no recall or relabelling is needed for stock already in the supply chain.

The list includes immunotherapy drugs such as pembrolizumab and durvalumab, along with osimertinib, denosumab, pegfilgrastim and bevacizumab, as set out in the NPPA’s October notification on PIB. The exercise builds on a 2019 pilot that capped margins on 42 anti-cancer drugs and cut prices of 526 brands by up to 90%.

Compliance from 9 November

Every retailer, dealer, hospital and institution must display the revised MRP list of anti-cancer medicines on its premises and website. State drug controllers will monitor compliance, and patients can check prices and report violations through the Pharma Sahi Daam app and the NPPA website.

The move comes after the Supreme Court questioned the Centre over the MRP gap in medicines and amid wider scrutiny of how drugs are priced, as seen in the AstraZeneca NPPA inhaler case earlier this month.

Why it matters for India’s pharma industry

Trade margin rationalisation shifts pricing pressure from manufacturers to the distribution chain, which has carried markups of over 100% on some cancer medicines. Media reports on 7 November said the NPPA is now studying anti-infectives, India’s top-selling therapy segment, as the next possible category for a margin cap. For drugmakers, distributors and hospital pharmacies, oncology pricing is set to become a template for wider reform.

Key takeaways

  • Trade margins are capped at 30% of MRP, saving patients about ₹3,502 crore a year.
  • 600 brands have cut prices by more than half, with one cut of 96.45%.
