Metropolis Healthcare Limited, one of India’s largest and most respected pathology networks, has released a nationwide analysis that signals an urgent shift in the country’s cardiovascular health profile. Based on 3.9 lakh lipid profile tests conducted over the past year, the study reveals that one in four Indians exhibits abnormal cholesterol levels, with low levels of HDL (‘good cholesterol’) emerging as the most consistent risk factor across age groups.
Key Findings at a Glance
- 35% of individuals presented with low HDL, a condition strongly associated with higher cardiovascular risk.
- 30% recorded elevated total cholesterol, while 33% showed abnormal triglyceride levels.
- 24% had borderline or high non-HDL cholesterol — an important marker of atherosclerosis.
- 25% demonstrated borderline to very high LDL cholesterol, underscoring the pervasiveness of lipid imbalance.
Early Onset Risk Among Young Adults
The most concerning trend is the early onset of lipid abnormalities. More than one in three adults aged 19–30 years were found to have low HDL levels, while 17% showed borderline high cholesterol — a pattern historically confined to older populations. This shift highlights how sedentary lifestyles, processed food consumption, stress, and poor physical activity are accelerating cardiovascular risks in India’s younger demographics.
Independent of Obesity
Another significant insight is that these lipid abnormalities often occur independent of obesity. Many seemingly healthy individuals — not classified as overweight — were found to have low HDL and high triglycerides. This reinforces the need for broader preventive health strategies beyond weight management, with a focus on comprehensive cardiac screening and lifestyle interventions.
Industry Response: Preventive Health at the Core
In line with the findings, Metropolis has rolled out the “Healthy Heart Meter” campaign to strengthen preventive healthcare. Through a simple five-question risk assessment, individuals are classified as Low, Medium, or High risk. The initiative links risk categorisation with targeted diagnostic recommendations — from a basic lipid profile for low-risk participants to the TruHealth Vital Package for those in higher risk brackets, offering a more holistic view of health.
Voices from Metropolis
- Surendran Chemmenkotil, Managing Director, Metropolis Healthcare Limited, emphasised the role of diagnostics in early intervention: “Raising awareness and enabling early detection of cardiovascular risks can significantly change outcomes. Initiatives like the Healthy Heart Meter are designed to provide individuals with actionable insights, empowering them to take timely preventive steps.”
- Dr. Kirti Chadha, Chief Scientific and Innovation Officer, Metropolis Healthcare Limited, highlighted the need for a preventive mindset: “The growing prevalence of lipid abnormalities among young adults should serve as a wake-up call. A simple lipid profile test can reveal silent risks long before symptoms appear. What we need now is a shift from treatment to prevention.”
Why It Matters for the Healthcare Ecosystem
For the B2B healthcare industry — including hospitals, insurers, and digital health platforms — these findings underscore a widening preventive care opportunity. With cardiovascular risks emerging earlier and beyond traditional obesity-linked markers, there is a pressing need for:
- Integration of regular lipid screening into wellness programs.
- Innovative insurance products reward proactive testing.
- Employer-led health initiatives targeting younger workforces.
- Collaborations between diagnostics, providers, and digital health players to promote sustained engagement in preventive care.

As the healthcare sector marks World Heart Day 2025, Metropolis’ study offers both a warning and an opportunity: India’s heart health challenge is deepening, but with timely diagnostics, collaborative industry efforts, and proactive public participation, the trajectory can still be reshaped.
